• Requesting a Certificate of Insurance (COI)

If you are required to provide a Certificate of Insurance to any entity for any reason, complete the following information to request the Myerstown Soccer Club COI.   INCLUDE A COPY OF THE CONTRACT or sections relating to insurance, indemnification and/or hold harmless agreements with this form.
 

• Game Reschedule Request Form
  This is for MSC Rec and ELCO United coaches to request a field for a makeup/rescheduled home game.
